I understand that a second opinion is a review of previously acquired images, is not an in-person consultation, does not establish ongoing clinical care and does not replace the assessment of my treating physician, who remains responsible for treatment decisions.

I understand that the quality of the report depends on the quality and completeness of the images provided, and that additional studies may be required.

I authorise the processing of my personal and health data for the purpose of this request, under Brazilian data protection law (LGPD), kept under medical confidentiality.
